Enroll Your Student Today!

If you are interested in enrolling your student in our school, here are the basic steps you need to take:

1. Fill out and sign a PVCS Enrollment Form.
   This Form consists of two documents (click to download)
   • 2010-2011 Form 1A
   • 2010-2011 Form 1B
2. A COPY of your child’s official birth certificate.
3. Provide a COPY of an up-to-date immunization record.
4. All withdrawal slips from your student’s previous school.
5. COPY of transcripts or last report card & school records.
6. COPY of standardized test scrores and/or individualized education plan, if applicable.
7. Call and make an appointment between the student and the School District Administrator.

Enrollment forms can be obtained in our school office Monday through Friday from 7:45 a.m. to 3:45 p.m

A uniform shirt must be purchased before a student may attend class.
