• - NCLB Eligibility (required for enrollment)
• - Official birth certificate
• - Up-to-date immunization record
• - Withdrawal slip from prior school
• - Transcripts or last report card
• - Standardized test scrores
• - Individualized Education Plan, if applicable.
• - ELL Assessments, if applicable.
• - Custody/Visitation court records, if applicable.

Applications received are reviewed weekly by the administration for available space. Upon acceptance, a uniform shirt will need to be purchased from the school office.
